Dharamshila Hospital Organized A Live Workshop on Composite Resection with Micro vascular Free Fibular Flap Reconstruction

A live workshop on Head and Neck Cancer Surgeries was organized at Dharamshila Hospital And Research Centre (DHRC). Worldwide Head and Neck cancers are sixth most common cancers in men. In India Head and Neck Cancers are 2nd most common among men and are responsible for 40% of new cancer cases detected annually. Even though 75% of Head and Neck Cancers are caused by tobacco and alcohol use, India has also witnessed a rise in the number of Head and Neck cancers in Non smokers. Due to increased tendencies for oral sex, men are getting infected with Human papilloma virus 16 which also is being considered as one of the causes of oral and oropharyngeal cancers.
According to Dr. S. Khanna, Executive Director, Dharamshila Hospital And Research Centre “Most cases of Head and Neck cancers are diagnosed at stage III and stage IVA. Head and neck Cancer Surgeries require complex infrastructure with a dedicated team. Due to lack of desired infrastructure, even now most of the Head and Neck surgeries are not being performed anywhere in North India. She also said that Dharamshila Hospital is performing 180 – 200 free flap reconstructions per annum which is the highest figure in India and offers more than 96% success rate. The credit for such high success rate goes to the team work of our dedicated and focused Head and Neck Surgical Oncologists, Plastic Surgeons, Anaesthesiologists, Critical care experts, Radiation Oncologists, Medical Oncologists, Onco-pathologists, Imaging Experts, Physiotherapists, Critical Care Nurses, technicians, Prosthodontics , speech therapist and high-end diagnostic, prognostic and therapeutic technologies”.
Dr. Anshuman Kumar and Dr. Mudit Agarwal, Head and Neck Surgical Oncologist demonstrated and explained the steps of composit resection of the cancerous tissues ensuring clear margins and removing all lymph nodes. After removal of the cancerous tissues, a large gap appeared which, was reconstructed by the plastic surgeons using microvascular flap from leg for jaw reconstruction by Dr. Ehsaan Siddiqui and Dr. Anshuman Kumar.
Dr. Anshuman Kumar explained that the objective of this high risk and challenging surgery is to ensure that patient is cancer free, gets close to normal appearances with restoration of eating, swallowing and voice. The august gathering of participants included Surgical Oncologists, Facio-maxillary surgeons, dental surgeons, ENT surgeons etc. During Surgery, participants were interacting actively with the Operating team and the moderator.
This was followed by a video of a patient, who had fully recovered from this surgery and shared his experience during treatment at DHRC. Afternoon session on video workshop was conducted by Dr. Mudit Agarwal. He showed videos of Co2 laser surgeries for organ and function preservation for early cancers of vocal cord and larynx (voice box). He also explained the advantages of endoscopic Co2 laser resection, where there is no external cut and surgery is done from inside the mouth. The major advantage of the procedure is that patient can be discharged next day and resumes work within few days. This procedure has minimal side effects with good voice preservation. Co2 laser surgeries at Dharamshila Hospital are a boon for patients of North India. If Co2 laser surgery is not possible, voice conservation open surgeries are performed, preserving the voice of a person.
